What telehealth is

Telehealth is the delivery of healthcare using secure electronic communication — questionnaires, messaging, phone, and video — instead of an in-person visit. Your care is provided by an independently licensed clinician licensed in your state.

Potential benefits

• Access to a licensed provider without travel or a waiting room.
• Faster follow-up and messaging between visits.
• Care coordination with a licensed dispensing pharmacy.

Potential limitations and risks

• No physical examination is performed, so some conditions cannot be diagnosed or ruled out remotely.
• Your provider relies on the accuracy and completeness of the information you supply; incomplete information can lead to an inappropriate or delayed decision.
• Technology failures may delay evaluation or treatment, and no electronic transmission is completely secure.
• Your provider may determine that an in-person evaluation, lab work, or referral is required before treatment can be considered.

Your rights

• You may ask questions about your care and about your provider's credentials.
• You may request an in-person referral instead of telehealth care at any time.
• You may withdraw this consent at any time by emailing support@vimorarx.com; withdrawal does not affect care already provided.
• Your health information is protected under applicable federal and state law and is shared only as described in the Privacy Policy.

No guarantee of outcome

No provider can guarantee a specific result. Treatment, if any, is prescribed only when the provider determines it is clinically appropriate. A provider may decline to prescribe.

Emergencies

Telehealth is not for emergencies. If you experience chest pain, difficulty breathing, severe abdominal pain, signs of an allergic reaction, thoughts of self-harm, or any other urgent symptom, call 911 or go to the nearest emergency department.

Acknowledgement

By completing an intake and receiving care through Vimorarx, you acknowledge that you have read and understood this consent, that you are at least 18 years old, and that you are physically located in a state where care is available.
